Job summary

Union Community Care is seeking a Community Health Resource Coordinator to assist patients in meeting social and behavioral goals. You will coordinate with medical, dental, and behavioral health teams to act as a social services case manager for vulnerable patients with complex needs including housing, insurance, and mental health services.

You will enroll adults and families in health insurance programs, conduct outreach to promote coverage, and document referrals in the electronic health

Qualifications

• Bachelor’s degree preferred or HS diploma with substantial relevant experience.
• 3+ years of relevant experience.
• Certified Application Counselor or Navigator required.
• Must communicate effectively in person and in writing and work collaboratively with community organizations, health center staff and clinicians.

Responsibilities

• Serve as an active member of medical and dental teams to coordinate social assistance referrals and ensure integrated care.
• Collaborate with individuals and families to create care plans based on trust, cultural humility, and social determinants of health.
• Provide social assistance including health insurance enrollment, housing, mental health referrals, and access to public benefits.
• Enroll children and adults in public and private health insurance.
• Conduct outreach to promote health insurance and enrollment.
• Provide language-appropriate services and translate/interpret documents.
