Frederick Health is seeking a Project Coordinator to support the Frederick County Transformation Collaborative, coordinating workplans, deliverables, and communications. The role tracks progress, maintains dashboards and logs, and ensures compliance with federal, state, and Frederick Health requirements.

You will work under the Director of Community Health, serving as an operational resource for healthcare providers, community-based organizations, and other partners.

Qualifikationen

• Bachelor's degree in a relevant field required.
• Minimum three years of project coordination or related experience.
• Experience coordinating multidisciplinary teams and programs.
• Experience facilitating meetings and maintaining project documentation.

Aufgaben

• Coordinate day-to-day execution of approved project workplans and deliverables.
• Track progress toward objectives, commitments, milestones, and grant activities.
• Maintain project schedules, dashboards, logs, and reporting calendars.
• Prepare meeting minutes and distribute agendas; record decisions and actions.
• Support operational readiness activities for Community Health Hub implementation.
