Frederick Health Care Management Assistant roles support the Integrated Care Management team by outreach and assessment during transitions in care, closing gaps in quality measures, and handling administrative tasks under the primary care team. You will collaborate with Frederick Health departments and community partners to improve patient outcomes.

This position emphasizes patient engagement, documentation in EHR, and coordination of care with physicians and care managers across primary care

Formación

• AA degree with 3–5 years clinical work experience preferred.
• Certified Medical Assistant or Certified Nursing Assistant certification preferred.
• Must have current CPR certification.

Responsabilidades

• Gaps in Care: outreach to attributed patients, generate orders/referrals, obtain records, schedule appointments.
• Access and utilize software systems to identify target patients and complete documentation.
• Outreach during care transitions to assess discharge instructions and ensure follow-up.
