Job summary

Frederick Health in Frederick, MD is seeking a part-time Project Coordinator to support the Frederick County Transformation Collaborative, a rural health initiative funded by the Maryland Rural Health Transformation Program.

The role coordinates workplans, partner communications, deliverables, meetings, data collection, documentation, and grant reporting to promote timely achievement of project objectives and compliance with applicable requirements.

Qualifications

• Bachelor's degree or equivalent combination of education and experience in healthcare administration or related field.
• Minimum three (3) years of project coordination or related experience in healthcare operations or population health.
• Experience coordinating multidisciplinary teams, programs, or initiatives.

Responsibilities

• Coordinates day-to-day execution of approved project workplans, implementation schedules, milestones, and assigned deliverables across the Collaborative.
• Tracks progress toward project objectives, partner commitments, implementation milestones, and grant-funded activities.
• Maintains project schedules, dashboards, action logs, risk and issue logs, decision logs, reporting calendars, and other approved project-management tools.
• Monitors due dates and dependencies; identifies delays or barriers and escalates to the Director of Community Health.
• Compiles routine project status information and supporting documentation for leadership and governance review.
• Supports operational readiness activities for Community Health Hub implementation, referral pathways, workforce placements, data infrastructure, and sustainability planning.
