Job summary

Asian American Center of Frederick (AACF) is seeking a Community Health Worker Coordinator to supervise CHWs, manage complex cases, and carry a reduced caseload. You will work with the Director of Community Health, a Nurse Practitioner, and IT staff to ensure high-quality referrals and follow-ups.

The role requires 5+ years in community health, MDH CHW certification (or eligibility), and strong coaching and communication skills. Occasional evenings and weekends may be required.

Qualifications

• Five or more years of experience as a Community Health Worker or similar frontline role.
• MDH CHW certification or eligible to certify within onboarding period.
• Experience coordinating with clinical and social-service partners.

Responsibilities

• Supervise and coach CHWs; lead team reviews and care huddles.
• Assign and balance caseloads; monitor follow-up timeliness.
• Oversee field observations; provide constructive feedback to CHWs.
• Document contacts, referrals, and outcomes in Salesforce.
• Support safety and wellbeing; escalate complex cases with clinical partners.
• Participate in hiring and performance evaluations.
