Job summary

Corewell Health is seeking a Supervisor of Community Health Programs for the Center for Wellness in Benton Harbor, MI. The role oversees Community Health Workers, Social Navigation, and front desk operations to deliver chronic disease prevention, health equity, and care coordination within Healthier Communities South.

Qualifications

• Bachelor's degree in community health, education, nursing, social work or related field is required.
• Preferred Master's degree in related field.
• 3 years of recent relevant community program development experience.
• Grant writing experience preferred.
• BI-lingual Spanish is preferred.
• MI RN or MSW licensure paths are preferred or required based on hire.

Responsibilities

• Supervise activities of staff engaged in planning and prioritizing care for individuals and populations.
• Recruit, schedule, and supervise program volunteers; maintain activity documentation.
• Coordinate referrals and ensure credentialing compliance; monitor service utilization.
• Coordinate program planning, implementation, and evaluation with community partners.
• Act as client advocate and liaison among clinical teams, partners, and operations.
• Develop support systems and coordinate programs with system-wide/community supports.
• Initiate process improvements and measure outcomes; make quality recommendations.
• Participate in hospital/community committees as needed.
