Qualifikationen

• Bachelor's degree in community health, education, nursing, social work or related field.
• 3 years of recent relevant community program development.
• Previous grant writing experience.

• Supervises Community Health Workers, Social Navigation staff, and front desk operations.
• Coordinates services across care settings to ensure a seamless participant experience.
• Develops, implements, and evaluates educational programs and teaching strategies.
• Recruits, schedules, and supervises staff and volunteers; maintains records.
• Ensures credentialing compliance and monitors utilization and referral patterns.
