Qualifications

• High school diploma required, Bachelor's degree preferred.
• Graduate of Medical Office Assistant Program or hold national Medical Assistant Certification (NCCA/ANSI) preferred.
• Bi-lingual language skills are a plus.
• Experience as Medical Assistant in primary or specialty care is preferred.

Responsibilities

• Promote access to services and assist community members in navigating health care and social services systems.
• Engage patients in community and home settings; assist with vitals and point-of-care testing.
• Identify social, medical, and financial barriers; provide health education to patients and community.
• Travel to patients' homes and community settings to discuss Community Health programs.
• Develop health and social management plans; coordinate with care teams and community resources.
• Maintain accurate EMR documentation and participate in team meetings.
