Qualifications

• High school diploma required; Bachelor’s degree preferred.
• Graduate of a recognized Medical Office Assistant Program or hold Medical Assistant Certification preferred.
• Experience as a Medical Assistant in Primary or Specialty Care preferred.
• Bi-lingual language skills are a plus.

Responsibilities

• Promote access to services and assist community members in navigating health care and social services systems.
• Engage patients in the community and home settings and collaborate with the Department of Emergency Services.
• Obtain vitals and perform point of care testing as needed.
• Deliver health education to patients and the community.
• Build connections between vulnerable populations and health care/system resources.
• Travel to patients’ homes and community events to discuss Community Health programs.
• Facilitate healthcare and social service system navigation for clients.
