Job summary

Oak Street Health is hiring a Community Health Worker to serve as a bridge between patients, the community, and medical systems. You’ll promote health literacy, coordinate care, and connect individuals with local resources, spending substantial time outside the center in patient-centered settings.

You will collaborate with Medical Social Workers, support care plans, and document interactions in the EMR while upholding HIPAA.

Qualifications

• At least 1 year in healthcare or social services.
• Strong oral and written communication abilities.
• Ability to travel across the community and work outside the center.
• US work authorization.

Responsibilities

• Establish and maintain relationships to coordinate patient needs.
• Manage patient referrals with the care team.
• Facilitate communication among patients, families and providers.
• Build relationships with local community organizations.
• Connect patients to housing, transportation, food and other resources.
• Assist patients with applications for benefits and resources.
• Promote goal setting to improve quality of life.
• Meet patients at Oak Street Health centers or in the community.
• Spend about half the time outside the center in community locations.
• Document interactions in the EMR with HIPAA compliance.
