Job summary

One Cross Health in Lebanon, KY is seeking a Community Health Worker (Full-Time) to conduct community outreach and provide health education in-person. You will help residents navigate the healthcare system, including Medicare, and promote wellness in the local community.

The role emphasizes working with individuals facing addiction and other health challenges, collecting data, and collaborating with healthcare providers on health promotion programs.

Qualifications

• Knowledge of Medicare.
• Strong communication and interpersonal skills.
• Understanding of medical terminology.
• Experience in addiction counseling or related fields (preferred).
• Ability to work independently and stay organized.

Responsibilities

• Conduct community outreach and provide health education.
