Job summary

Grace Health is seeking a Community Health Worker to serve as a frontline liaison between patients and health/social services in Kentucky. The role emphasizes chasing certification, educating patients, and coordinating with care teams to improve access and quality of care.

The CHW will perform outreach, provide informational support, and maintain accurate records while following Grace Health policies and confidentiality standards.

Qualifications

• Basic communication and interpersonal skills
• Good verbal skills are required
• Valid driver’s license is required

Responsibilities

• Complete a certification in Community Health Worker services.
• Support patients by following provider instructions and assessing information needs; provide information and referrals.
• Serve as liaison between patients, families, and service providers; report relevant conditions.
• Collaborate with multi-disciplinary teams to establish and implement action plans; connect patients to services.
• Maintain precise records per protocol.
• Participate in continuing education programs and peer support activities.
• Adhere to Grace Health policies; maintain attendance and documentation.
• Maintain patient confidentiality and multitask efficiently.
