Job summary

The MetroHealth System is seeking a Community Health Worker to support patients within assigned programs by promoting health, reducing disparities, and improving access to care. You will perform outreach to individuals and communities, connect them to services, and build trusted relationships with patients, families, and care teams.

Applicants should have a high school diploma or GED, at least one year of community-based experience, CHW certification within a year, and strong communication

Qualifications

• High school diploma or GED is required.
• One year of experience with clients in a community-based setting.
• Community Health Worker certification within one year of hire.
• Life experiences reflecting the target population served.
• Reliable transportation to designated locations.
• Ability to work with diverse populations, organizations, and staff.
• Ability to work independently and as part of a team.
• Excellent verbal and written communication skills.
• Strong interpersonal, administrative, and organizational skills.
• Proficiency with personal computers (MS Office, etc.).
