Job summary

Missouri Highlands Health Care is seeking a Community Health Worker (non-exempt) to work primarily in the community with specific populations. You will collaborate with medical providers, primary care teams, and other agencies to improve patient care and outcomes.

The role focuses on assisting patients with health management plans, Medicaid applications, and navigating community resources while addressing barriers to care and promoting self-management of chronic conditions.

Qualifications

• High school diploma or GED required.
• BLS certification required.
• 3 years health/social services experience and/or education preferred.

Responsibilities

• Convey the purposes and services of a program to the user population and the impact.
• Help patients develop health management plans and goals.
• Assist patients in Medicaid application and submissions.
• Follow-up with health management/care plans with patients and providers.
• Coach patients in managing chronic health conditions and self-care.
• Assist clients in understanding care plans and instructions.
• Document activities and results per policies and procedures.
• Work collaboratively within a team and build positive relationships.
• Help clients access resources, schedule appointments, and complete applications.
• Coordinate services between providers and community resources.
