About the Role

Our client is looking for a proactive and empathetic Community Health Navigator to join their team, serving the residents of Welkom and surrounding areas in the Free State . This role is critical in bridging the gap between community members and accessible healthcare services. You will assist individuals and families in understanding their health needs, navigating the complexities of the healthcare system, and connecting them with appropriate medical providers, social services, and wellness programs. The ideal candidate is knowledgeable about local health resources, possesses excellent communication skills, and is committed to improving health outcomes for all community members.

Key Responsibilities

• Assist community members in identifying their health needs and accessing appropriate healthcare services.
• Provide information and support regarding preventative health measures and chronic disease management.
• Help clients navigate insurance coverage, appointment scheduling, and transportation to medical facilities.
• Connect individuals with social support services, mental health resources, and community wellness programs.
• Conduct health education outreach sessions within the community to promote healthy lifestyles.
• Maintain accurate records of client interactions and outcomes, ensuring confidentiality.

Requirements

• Experience in community health, social work, or a related field.
