Job summary

Northeast Florida Healthy Start Coalition is seeking a Coordinated Intake & Referral Community Health Worker to support CI&R services from the Community Programs Office. This full-time, non-exempt role focuses on linking pregnant and at-risk families to home visiting and related resources in designated Jacksonville zip codes.

Responsibilities include outreach, intake assessments, referrals, and data entry, with a strong emphasis on culturally competent support and collaboration with community

Qualifications

• High school diploma required with two years of community-facing experience.
• Strong customer service and communication skills.
• Spanish language ability a plus.

Responsibilities

• Advocate or liaison in the community, connect with high-risk families to navigate home visiting, healthcare, and social services.
• Net­work with community providers to promote CI&R referrals and recruit eligible participants.
• Conduct initial intakes and maintain participant records in the Well Family System.
• Deliver outreach, presentations, and information about CI&R services in targeted zip codes.
