Job summary

UF Health seeks a Community Health Associate to deliver direct community health services through Care Connect to vulnerable populations. You will engage residents, identify unmet health and social needs, and connect them to available resources via Care Connect+.

The role supports Care Connect+ with crisis intervention and client-centered assessments, spanning programs across Florida. Required: High School Diploma; 0–1 year experience; AHCA Level 2 screening.

Qualifications

• High School Diploma or equivalent required.
• Preferred Associate’s or Bachelor's degree in Social Work, Public Health, Healthcare Administration, Psychology, or related field.
• 0–1 year of experience working with vulnerable populations.
• Must pass AHCA Level 2 screening.
• Strong communication and interpersonal skills; calm under pressure.

Responsibilities

• Provide direct community health services through Care Connect to vulnerable populations.
• Engage clients, identify needs, and connect to resources addressing social determinants of health.
• Coordinate with Care Connect+ network to improve access to services.
• Maintain client-centered assessments and crisis intervention when needed.
