Responsibilities

• Engage, motivate, and advocate community members to build knowledge and skills for self-directed change and community development.
• Builds and maintains collaborative relationships with the community, clinic staff, and partners.
• Provides education to individuals or groups on health topics including health promotion, testing, vaccination, and support services.
• Provides information and referrals to community resources to improve health and address social needs.
• Assists patients in accessing health services and enrollment forms.
• Collects data and provides feedback to evaluation teams and program management.
• Uses assigned technology to complete job functions and documents confidential information.
• Reports on program implementation, client outcomes, and stories with highlights or recommendations.
• Performs other duties as required by lead care manager and department needs.
