Responsibilities

Conduct screenings to identify NMDOH and document risk levels in EMR.
Use motivational interviewing to explore needs and connect to internal or community resources.
Verify referrals with partner agencies to ensure alignment with needs and eligibility.
Schedule and follow up on clinical and social service appointments; document outcomes.
Perform follow-ups on referrals to ensure service completion and patient satisfaction.
Participate in community activities to identify new resources and represent Memorial Hermann.
Provide support at other Memorial Hermann locations as needed.
Adhere to policies, maintain safety, and support professional growth and education.
Demonstrate compassionate, personalized, efficient care experiences for all community members.
