Qualifications

• Education: High school diploma or GED required.
• Licenses or Certifications: CHW certifications (TDSHS) include CHW, CHW-I, CHW-E, CHW-T.
• Experience or knowledge in healthcare, social services or community outreach is a plus.
• Bilingual in Spanish preferred.
• Strong communication, organization, and rapport-building skills.

Responsibilities

• Conduct screenings to identify NMDOH needs and document risks in the EMR.
• Use motivational interviewing to explore needs and connect to resources.
• Verify referrals with partner agencies to ensure appropriate services.
• Schedule and follow up on clinical and social service appointments.
• Engage in community activities and represent Memorial Hermann at events.
• Provide cross-location support as needed and adhere to policies.
