Job summary

Pioneers Memorial Healthcare District in Brawley, CA is seeking a Community Health Worker to help patients navigate services, access resources, and adopt healthy behaviors within the Population Health program.

The CHW will build relationships with the community and partners, provide outreach and education on health topics, refer to care and social services, and maintain confidential information while using appropriate technology.

Qualifications

• High school graduate or equivalent required.
• Medical Assistant Certification preferred.
• Fluent or functional Spanish preferred.
• Excellent communication and interpersonal skills.
• Ability to use basic computer applications including Excel.

Responsibilities

• Engage, motivate, and advocate community members to build knowledge and skills for self-directed change.
• Provide outreach and education on health topics and resources to individuals or groups.
• Offer information and referrals to community resources to address social needs.
• Assist patients in accessing appointments and health services.
• Collect data and report findings to evaluation teams.
• Maintain confidentiality and use assigned technology to perform tasks.
• Document program activities and client stories for program management.
• Provide ongoing support and services for clients as needed.
