Job summary

The Wright Center for Community Health and Graduate Medical Education is seeking a Community Health Worker to collaborate with medical providers and social services to coordinate care for patients. The CHW will conduct home visits, health screenings, and community events as part of outreach and support.

Responsibilities include documenting encounters in EMR, educating patients on care plans, and connecting patients with community resources.

Qualifications

• High school diploma or GED; at least 3 years of experience directly related to the duties.
• Current BLS certification is required.
• Bilingual highly desired and strong interpersonal skills are important for community engagement.
• Knowledge of community agencies and resources is essential.
• Ability to plan, implement, and evaluate individual patient care plans.

Responsibilities

• Assist patients in their homes, community, or health center setting.
• Communicate program purposes and potential impact to patients.
• Identify socio-economic issues affecting health and develop care and social management plans.
• Document encounters in EMR and submit monthly reports.
• Educate patients on proper use of the Emergency Department and self-care of chronic conditions.
• Coordinate services between providers and patients; track care plan objectives.
• Attend weekly huddles and participate in extended access hours as needed.
