Job summary

Southeastern Michigan Health Association (SEMHA) seeks a Community Health Worker to join Wayne County Healthy Communities in Wayne, MI. The CHW will support patients and community members with outreach, health education, care coordination, insurance enrollment assistance, and access to community resources.

The role emphasizes collaboration with clinical staff, health plans, and partners to improve access to care and health outcomes while maintaining HIPAA compliance and program requirements.

Qualifications

• High school diploma or equivalent REQUIRED.
• MiCHW certification preferred.
• CAC certification preferred at hire.
• Bachelor’s degree in health sciences, business, clinical studies, or related field preferred.

Responsibilities

• Educate and engage patients on preventive screenings, chronic disease management, and available center services.
• Coordinate appointments, testing, referrals, and follow-up; identify barriers to care and connect with community resources.
• Conduct SDOH screenings and support referrals between faith-based organizations, community partners, and FQHC services.
• Conduct targeted outreach to Health Plan members with open HEDIS gaps or high ED utilization.
• Review outreach lists, maintain assigned patient panels, and assist with PCP changes and panel accuracy.
• Document outreach, education, referrals, enrollment assistance, and outcomes in designated systems; submit monthly reports.
