Job summary

Honor Community Health in Michigan is hiring a Community Health Worker to join an interdisciplinary care team delivering integrated primary, behavioral health, and social support services to underserved populations.

You will engage patients, coordinate six core services, document in EHR, and help address barriers to care with compassion. The role requires a high school diploma, CHW certification within 12 months, and two years of related experience, with strong computer skills.

Qualifications

• High School Diploma or equivalent is required.
• Current MiCHWA Community Health Worker certification or ability to obtain within 12 months of hire.
• Two years of experience directly related to the duties or lived experience in the communities Honor Community Health serves.
• Proficient computer skills including Microsoft Office, EPIC, and CHAMPS.

Responsibilities

• Deliver integrated care management and coordination to eligible beneficiaries.
• Assist in six core services: care management, coordination, health promotion, transitional care, individual and family support, and referrals.
• Document care details in the Electronic Health Record and population health management tool.
• Communicate effectively and manage multiple tasks in a fast-paced environment.
• Foster a compassionate, patient-centered experience.
