Southeastern Michigan Health Association (SEMHA) in Wayne, MI, is seeking a compassionate Community Health Worker to join the Wayne FQHC Program. The CHW will help increase access to health services through outreach, education, and peer support within an inter-disciplinary clinical care team.

The role emphasizes outreach, SDOH screening, referrals, and coordination with PCPs. A part-time schedule, HIPAA compliance, and strong communication skills are essential.

Competenze

• High school diploma required.
• Bachelor’s degree in health sciences preferred.
• MICHW certification preferred.
• Experience in community work or healthcare strongly preferred.
• Strong communication, organizational and interpersonal skills.
• Ability to multitask in a fast-paced environment and advocate for patients.
• Proficiency in Microsoft Office preferred.
• Valid driver’s license and reliable transportation required.

Mansioni

• Conduct patient outreach and provide education on preventive screenings and chronic disease management.
• Address HEDIS care gaps and engage high emergency department utilizers to connect with their PCP.
• Perform Social Determinants of Health (SDOH) screenings and link patients to appropriate community resources.
• Coordinate referrals, assist with scheduling, and help remove barriers to care.
• Maintain accurate documentation, outreach tracking, and monthly reporting for Health Plans.
• Participate in community outreach events and support site visit preparation (e.g., HRSA, AAAHC).
• Maintain HIPAA compliance and confidentiality at all times.
• Support patient panel management and other duties as assigned.
