Job summary

Southeastern Michigan Health Association (SEMHA) is seeking a Community Health Worker to join Wayne County Healthy Communities in Wayne, MI. The CHW will support patients and community members through outreach, education, care coordination, insurance enrollment assistance, and connections to community resources.

The role collaborates with clinical staff, health plans, and community partners to address barriers to care, improve access to services, and promote positive health outcomes.

Qualifications

• Excellent written and verbal communication required.
• Ability to manage multiple projects and priorities.
• Attention to detail and compassion toward patients.
• Ability to work with diverse communities and partners.
• Experience in community outreach or health care preferred.

Responsibilities

• Educate patients on preventive screenings and service utilization.
• Coordinate appointments, referrals, and follow-ups; identify barriers to care.
• Conduct SDOH screenings and connect to resources.
• Outreach to Health Plan members and address care gaps.
• Document interactions and outcomes in designated systems.
• Assist with Medicaid/Marketplace enrollment and CAC certification.
