Job summary

Southeastern Michigan Health Association seeks a compassionate Community Health Worker to join the Wayne FQHC Program in Wayne, MI. You will support outreach, education, and peer assistance to improve access to care.

The role emphasizes addressing care gaps, coordinating referrals, and maintaining HIPAA compliance while supporting site visits by health plans and HRSA. Part-time, non-exempt, with benefits.

Qualifications

• High school diploma required.
• Bachelor’s degree in health sciences preferred.
• MICHW certification preferred.
• Experience in community work, education, or healthcare strongly preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to multitask in a fast-paced environment and advocate for patients.
• Proficiency in Microsoft Office preferred.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Conduct patient outreach and provide education on preventive screenings and chronic disease management.
• Address HEDIS care gaps and engage high emergency department utilizers to connect with their PCP.
• Perform Social Determinants of Health (SDOH) screenings and link patients to community resources.
• Coordinate referrals, assist with scheduling, and help remove barriers to care.
• Maintain accurate documentation, outreach tracking, and monthly reporting for Health Plans.
• Participate in community outreach events and support site visit preparation (e.g., HRSA, AAAHC).
• Maintain HIPAA compliance and confidentiality at all times.
• Support patient panel management and other duties as assigned.
