Job summary

Carilion Clinic seeks a Community Health Worker to assess patient needs and help navigate inpatient, community-based, and clinic services for at-risk individuals. You will engage using trauma-informed approaches and coordinate ongoing support with SMART goals.

You will accompany patients to appointments, advocate for access to care, and maintain thorough documentation in the hospital's EMR. The role lasts about six months of open care and requires strong collaboration with staff.

Qualifications

• High School diploma or equivalent required.
• 2 years of general work experience.
• AHA BLS-HCP must be obtained and maintained.
• Excellent interpersonal and communication skills; reliable transportation, valid driver’s license and proof of insurance.

Responsibilities

• Assists patients with accessing inpatient, community, and clinic-based services.
• Uses person-centered/trauma-informed approaches to engage and identify needs.
• Provides support and coordination with SMART goals and resource navigation.
• Accompanies patients to appointments to advocate and promote recovery and wellness.
• Maintains documentation of workflow and patient interactions in the electronic medical record.
