Job summary

Atrium Health is hiring a Community Health Worker to support patients and community members in navigating the healthcare system. The role emphasizes education on healthy behaviors and collaboration with medical providers and social services to facilitate access to resources.

This full-time position (40 hours/week) requires a High School Diploma or GED, 1–2 years of experience, and a valid driver’s license. Expect travel to client homes and community sites, with occasional evenings or weekends.

Responsibilities

Provide health education and prevention programs to clients.
Maintain client records with confidentiality.
Act as liaison between clients and healthcare providers.
Conduct follow-ups to ensure client needs are met.
Conduct home visits and community events to support care plans.
Help clients access community resources (housing, food, prenatal classes, etc.).
Assist with understanding care plans and instructions.
