Job summary

White House Clinics is seeking a Community Health Worker to support the primary care team in helping patients understand chronic conditions and adopt healthy lifestyles. You will coordinate care across settings, assist with education, and link patients to community resources to overcome barriers to health.

The role emphasizes teamwork, documentation in the EHR, and a patient-family centered approach. Evening/weekend work may be required as part of a flexible schedule.

Qualifications

• Minimum Education: High School Diploma or GED.
• Bachelor degree in Social Work, Health Promotions or Licensed Practical Nurse preferred.
• Strong verbal and written communication skills are required.

Responsibilities

• Participate as a key member of the care team, including morning huddles and team-based activities.
• Document conversations with and/or relating to patients in the electronic health record.
• Assist with other duties or in other departments as instructed by supervisor.
• Assess health literacy and tailor health education interventions.
• Receive health education referrals and complete approved curricula with patients.
• Assist patients in setting self-management goals using motivational interviewing.
• Monitor progress and coordinate care across settings; advocate for patients.
• Coordinate communication after inpatient discharge and/or ED visits.
• Provide navigation to community resources (food, housing, transportation, etc.).
• Ensure follow-up and track test results and medication management with the team.
