Job summary

CS CORE Services in Chattaroy, WV, is seeking a Community Health Worker and Medical Assistant to support clinic operations and patient care. The role blends outreach, education, and coordination to improve access to care for underserved populations.

You will document in the EHR, collect vitals, coordinate referrals, and assist telemedicine across Mingo and surrounding counties. A high school diploma, WV driver’s license, CPR/First Aid, and strong communication are required.

Qualifications

• High School Diploma or equivalent required.
• Must have CPR and First Aid certification.
• Two years of successful office or related work experience preferred.

Responsibilities

• Document patient information in the EHR.
• Complete intakes for nurses in the EHR.
• Record patient information and basic information about current and previous conditions.
• Measure and record vital signs (BP, height, weight, pulse, SpO2, etc.).
• Follow up on referrals and other patient appointments as needed.
• Schedule appointments with outside providers or for testing and follow up on outcomes.
• Prepare examination rooms and patients for examination.
• Collect specimens as needed.
• Prepare lab requisitions and label/store specimens.
• Ensure confidentiality of patient information and medical records.
• Assist telemedicine providers with recording vitals and documentation.
• Enter data electronically into clinical recordkeeping and billing systems.
