Job summary

The Wright Center for Community Health is seeking a Community Health Worker to support patients with care coordination, connect to resources, and provide education, including home visits and health screenings as needed.

The CHW will work closely with medical providers and social services, document encounters in the EMR, and help patients navigate insurance and appointments while supporting outreach efforts.

Qualifications

• High school diploma or GED with at least 3 years of related experience.
• Current BLS certification.
• Complete AHEC CHW training within 12–15 months of hire.
• Pennie Assister training certification within 90 days of employment.
• Bilingual highly desired.

Responsibilities

• Coordinate home visits and community outreach as part of patient care.
• Educate patients on program purposes and impact on wellbeing.
• Identify socio-economic barriers and develop health/social management plans.
• Document encounters in EMR and submit monthly reports.
• Educate on ED usage and alternatives; coach self-care.
• Facilitate access to community resources and services.
• Coordinate services between providers and patients; monitor care plans.
• Attend weekly huddles and extended hours rotation.
