Western Sierra Medical Clinic, Inc. is seeking a Community Health Worker to advocate for patients, coordinate care, and connect individuals with community resources. The CHW will operate across diverse settings to support high-quality, patient-centered care.

The role emphasizes outreach, enrollment assessments, and collaboration with a multidisciplinary team to address social determinants affecting health outcomes. Certification may be provided by WSMC if not already held.

Formación

• CHW certification preferred; willingness to obtain certification.

Responsabilidades

• Advocate for patients and help them achieve their best health.
• Be a trusted member of the community and serve as a liaison between patients, clinic and community resources.
• Conduct outreach and enrollment assessments.
• Create care plans reflecting patients’ needs and connect them to resources.
• Interface with clinic staff to ensure care coordination and documentation standards.
