Job summary

Signature Healthcare in Brockton, MA is seeking a Community Health Worker to support care coordination and smooth transitions for patients and families within the Patient Centered Medical Home team.

The CHW will collaborate with internal and external entities, act as a liaison between patients and the care team, and help close care gaps by connecting to community services and educating patients and caregivers.

Qualifications

• High School Diploma required; an Associates degree preferred.
• Experience as Medical Office Coordinator or Medical Assistant in an ambulatory setting preferred.
• Proficiency with Microsoft Office and general knowledge of electronic medical records.

Responsibilities

• Coordinate care by removing barriers and ensuring smooth transitions for patients and families.
• Liaise between patient and care team to address patient needs.
• Identify and close care gaps; connect to community services; support patient and family.
• Communicate effectively, build relationships, show compassion, and exercise good judgment.
• Familiar with community resources; work as part of a team; self-driven.
• Familiarity with healthcare systems and settings.
