Job summary

Brockton Neighborhood Health Center is seeking a Community Health Worker to support individuals with complex or chronic conditions by bridging care gaps and addressing social determinants of health. The role requires outreach to patients in diverse settings, including community, home, or health center locations, with occasional telephonic contact.

The CHW will collaborate with program leaders and care teams to schedule appointments, coordinate referrals, and provide culturally sensitive coaching

Qualifications

• Bilingual ability is preferred.
• Strong communication and collaboration with care teams.
• Experience with chronic or behavioral health populations is desirable.

Responsibilities

• Conducts outreach to engage patients and caregivers in care management.
• Develops outreach plans with team to address individual and community needs.
• Coordinate referrals, appointments, and follow-up within the care team.
