Job summary

Qualifications

Responsibilities

• Meet with care team to plan patient care and share information.
• Outreach to engage patients in preventive care and close gaps.
• Assist with entry, navigation, referral, and tracking of clinical needs and supports.
• Provide culturally appropriate health education and self-management support.
• Coordinate chronic disease management and teach self/home monitoring.
• Support medication adherence and problem-solve daily barriers.
• Co-lead health education groups and help implement wellness initiatives.
• Identify community resources and facilitate access to services.
