Job summary

Point32Health is seeking a Community Health Worker to support members in managing health goals within the Integrate Care Management team. The role emphasizes assessments, health education, and navigation of benefits to address social determinants of health and barriers to care.

Ideal candidates have CHW certification or willingness to obtain it, plus a bachelor’s in an allied health field and 3–5 years’ healthcare experience, including care coordination and advocacy.

Qualifications

• Credentialed by the Board of Certification of Community Health Workers (CHW certificate) or willing to obtain with assistance within one year of employment.
• Bachelor’s degree in an allied health discipline (e.g., Health Education, Public Health, Social Work) preferred.
• 3–5 years’ experience as a Community Health Worker in healthcare, with care coordination, advocacy, and community-based visits.

Responsibilities

• Conduct regular telephonic and in-person member assessments for Social Determinants of Health (SDoH) and ensure comprehensive care plans.
• Provide culturally appropriate health education and outreach in community settings.
• Bridge the gap between communities and health services to improve knowledge and self-reliance.
• Identify gaps in Health-Related Social Needs (HRSN) affecting care.
• Help members navigate state/federal benefits and access community resources.
