Job summary

Qualifications

• High school graduate or equivalent.

Responsibilities

• Establishes trusting relationships with patients and families.
• Identify socio-economic issues affecting health and develop plans.
• Assist patients with understanding health insurance options.
• Coordinate services between providers and patients.
• Connect patients to community resources including housing, food, education, and employment.
• Document encounters in the patient record and maintain comprehensive files.
