Job summary

MultiCare Health System seeks a Community Health Worker to join an interdisciplinary care team. The role focuses on care coordination, educating patients about community resources, and improving population health.

The position requires a High School Diploma or equivalent and 2 years of health care related experience, plus a valid Washington driver's license. Travel to community partners may be required and prior CHW experience is preferred.

Qualifications

• High School Diploma or equivalent is required.
• At least 2 years of health care related experience preferred.
• Current Washington State Driver's License with a safe driving record as per MHS standards.
• Experience in utilization management, community resources, and/or financial and quality issues affecting patient care preferred.
• Community Health Worker experience preferred.

Responsibilities

• Coordinate care across primary, specialty, urgent care, or other sites as appropriate.
• Engage patients via outbound communication to schedule appointments and encourage action.
• Help patients access community resources and schedule appointments.
• Establish contact with community partners; travel may be required.
• Develop tools to educate peers on resources and patient advocacy.
• Participate in community health fairs and events.
• Educate the care team on available resources.
• Participate in quality improvement activities.
