Commonwealth Care Alliance is seeking a Community Health Worker (CHW) to join its Care Delivery team in Massachusetts. You will support care delivery and coordination for high-risk patients, addressing social determinants of health and connecting patients with community supports.

You will conduct visits in person and virtually, document in English, and work with an interprofessional team including PCPs. Strong communication, organization, and experience in community-based care are required; CHW

Competenze

Associates degree required; bachelor’s preferred.
Experience in community-based care or outreach.
Familiarity with electronic medical records (eCW) ideal.

Mansioni

Deliver care to CCA’s most complex patients, providing care management and care coordination.
Engage in regular assessments of Social Determinants of Health (SDOH).
Conduct visits in person, telephonically, or virtually at scheduled intervals.
Provide health education and coaching to promote wellness and reduce health risks.
Collaborate with PCPs and interprofessional care team to coordinate care plans.
