Job summary

St. Bernards Healthcare in Jonesboro, AR seeks a Community Health Worker to serve as a patient-facing member of Population Health and Value-Based Care, bridging patients with providers, care managers, and community resources.

The CHW conducts culturally responsive outreach, provides health education, aids resource navigation, supports appointment scheduling, reinforces care plans, and follows up to improve engagement and reduce barriers to care.

Qualifications

• High school diploma or equivalent required
• Experience in health care, community health, social services, patient navigation, care coordination, or related field preferred
• Completion of a recognized CHW training/certification program preferred
• Strong interpersonal and communication skills
• Ability to establish rapport with diverse populations
• Knowledge of local community resources preferred
• Ability to maintain confidentiality and professional boundaries
• Basic computer and electronic health record skills
• Ability to work independently while functioning within an interdisciplinary team
• Ability to travel within the assigned service area when required

Responsibilities

• Engages high-risk patients with outreach, health education, resource navigation, appointment support, and care-plan reinforcement
• Provides follow-up to improve patient engagement and reduce barriers to care
• Works collaboratively with nurses, social workers, physicians, care managers, and other team members
• Supports organizational goals related to quality, patient experience, and health equity
• Must be able to travel within the service area as required
