Job summary

Saint Bernards Medical Center is seeking a Community Health Worker to serve as a patient-facing member of the Population Health team. You will function as a bridge between patients, healthcare providers, care management staff, and community resources.

You will engage high-risk patients, provide culturally responsive outreach and health education, navigate resources, support appointments, reinforce care plans, and follow up to improve engagement and reduce barriers to care.

Qualifications

• High school diploma or equivalent required.
• CHW training or certification preferred.
• Healthcare, community health, social services, patient navigation, care coordination or related field experience preferred.
• Strong interpersonal and communication skills.
• Ability to establish rapport with diverse populations.
• Knowledge of local community resources preferred.
• Ability to maintain confidentiality and professional boundaries.
• Basic computer and electronic health record skills.
• Ability to work independently while functioning within an interdisciplinary team.
• Ability to travel within the assigned service area when required.

Responsibilities

• Bridge between patients, healthcare providers, care management staff, and community resources.
• Engage high-risk patients and address social determinants of health.
• Provide culturally responsive outreach, health education, resource navigation, and appointment support.
• Reinforce care plans and follow up to improve patient engagement.
• Collaborate with nurses, social workers, physicians, care managers, behavioral health professionals, and dietitians.
