Responsibilities

• Refers and links patients to a medical home for preventive health care and coordinates basic social support.
• Assists patients in navigating the health system and connects to community resources for housing, food, job placement, and education.
• Collaborates with the care team to enroll patients in programs and coordinate care while respecting cultural practices.
• Engage with patients to set goals using motivational interviewing techniques.
• Adhere to performance productivity standards to meet department goals.
