The Community Health Worker (CHW) at St.-Bernards-Healthcare serves as a patient-facing member of the Population Health team, bridging patients with care providers and community resources.

They engage high-risk or socially affected patients, provide culturally responsive outreach and education, assist with appointments, and reinforce care plans to improve engagement and reduce barriers to care.

Formación

• High school diploma or equivalent required.
• Healthcare experience preferred.
• Completion of a recognized CHW training/certification program preferred.
• Strong interpersonal and communication skills.
• Ability to work independently while functioning within an interdisciplinary team.
• Ability to travel within the assigned service area when required.
• Basic computer and electronic health record skills.

Responsabilidades

• The CHW serves as a patient-facing bridge between patients and healthcare providers.
• Provides outreach, health education, and resource navigation to patients.
• Supports appointments and reinforces care plans; follows up to improve engagement.
• Works collaboratively with nurses, social workers, physicians, and care managers.
• Supports organizational goals related to quality, equity, and access to care.
