Job summary

PFH Preferred Family Healthcare seeks a Community Health Worker in Hannibal, MO to bridge patients with care teams and community resources. You will support navigation of healthcare, behavioral health, and social services, conduct outreach, and promote awareness of local resources.

The role requires a HS diploma, driver license, and 2+ years in related fields; a bachelor’s degree in health sciences or related area is preferred. Excellent communication and empathy are essential.

Qualifications

• High School diploma or equivalent required; valid driver's license.
• Bachelor's degree in Health Sciences, Social Work, Public Health, or related field preferred.
• Two years of experience in community health, patient navigation, healthcare outreach, human services, case management, or related field.

Responsibilities

• Care Coordination: support navigation of healthcare, behavioral health, and social services; follow up via calls, home visits, and community visits.
• Community Outreach: educate on resources, engage individuals, participate in events, promote culturally responsive care.
• Resource Navigation: connect patients with services, assist with insurance options and enrollment, provide culturally appropriate support.
• Documentation: maintain accurate records and support quality initiatives and projects.
