Qualifications

• At least 1 year of health care experience serving underserved or special needs populations.
• CHW certification may be required for certain states.
• Valid driver's license, reliable transportation, and adequate auto insurance for job travel.
• Ability to multitask while speaking with members.
• Excellent customer service skills.
• Strong organizational and time-management skills.
• Excellent verbal and written communication skills.

Responsibilities

• Engage with members as an advocate and resource to support health care needs.
• Collaborate with health care services team providing non-clinical paraprofessional duties in the field.
• Help members navigate health plan benefits and schedule appointments.
• Assist members in accessing social services and community resources.
• Provide outreach to locate and support disconnected members with special needs.
• Conduct research to locate hard-to-reach members using available data.
• Participate in outreach activities, including telephonic and in-person.
• Guide members to maintain Medicaid eligibility and other financial resources.
