Job summary

SAC Health is seeking a Community Health Worker, ECM to support patients and families with education about diseases and to navigate the healthcare system. You will coordinate referrals, arrange transportation, and track outcomes in the EHR, with a focus on language-appropriate resources for the community in San Bernardino.

You will conduct intake interviews, enroll in programs like Sliding Fee, and collaborate with community organizations to improve access to care.

Qualifications

• Education: High School Diploma or equivalent; AA preferred.
• Current CPR/BLS certification (AHA or Red Cross). EPIC certification for the module hired into.
• California driver's license and auto insurance required.
• 3+ years of experience in a community-based setting or related field.

Responsibilities

• Assist patients navigating the healthcare system and connect to community resources.
• Engage in community outreach and home visits to coordinate referrals and services.
• Coordinate communication with primary care teams and patients.
• Document metrics, outcomes, and health updates in the EHR; support data reporting.
