Job summary

Circle the City in Phoenix, AZ is seeking a Patient Navigator to work with the Integrated Health Care team to address social determinants of health and improve access to care and outcomes.

The role involves assisting with scheduling, transportation, housing, and connections to substance use and mental health services, while ensuring HIPAA compliance and strong teamwork across health centers and outreach teams.

Qualifications

• Must be 21 years of age.
• High school diploma or GED required.
• Spanish speaking preferred.
• Strong interpersonal skills with an ability to work effectively with others.
• Experience with culturally diverse, chronically homeless, and medically needy individuals preferred.
• Demonstrates excellent oral and written communication skills.
• Must demonstrate critical thinking, problem-solving, organizational and time management skills.
• Possess and maintain a valid Arizona driver’s license.
• Requires a current Arizona Department of Safety Clearance Card.
• Basic Life Support (BLS) Certification.

Responsibilities

• Refers and connects the patient to unmet social determinants of health such as healthcare, shelter, housing, services.
• Assesses eligibility for DES benefits and coordinates enrollment with uninsured patients.
• Assists patients in understanding care plans and documents progress toward care plans.
• Provides education and support to patients with chronic health concerns.
• Provides individualized care coordination and patient navigation.
• Collaborates with the care team to remove barriers to clinic delivery.
• Participates in team meetings, huddles, and quality improvement projects.
• Maintains positive relationships with patients, providers, and staff to ensure outcomes.
• Understands community needs and current events affecting care.
• Serves as a community referral resource within the agency.
