Job summary

Cityblock Health is seeking a Care Coordinator to support members with complex medical and behavioral health needs in Wake/Raleigh and surrounding communities.

You will work with an interdisciplinary team, conducting assessments, developing care plans, and coordinating services to address social determinants of health. A valid driver’s license is required, and the role offers health insurance and benefits.]

Qualifications

• Experience: 2+ years in care coordination, community health, health promotion, or a related field.
• Strong problem solving and sound judgment; know when to collaborate and elevate clinical concerns.
• High quality written, verbal and presentation communication; experience with documentation.
• Must have a valid unrestricted driver’s license.

Responsibilities

• Collaborate with RN Care Managers to support members with complex medical and behavioral health needs.
• Manage a caseload of members, identifying and addressing social, behavioral, and environmental barriers to optimal health.
• Develop and implement care plans with members, addressing social needs and supporting care goals.
• Conduct self-efficacy and condition-specific assessments, including behavioral health screenings (PHQ-9, GAD-7, AUDIT, DAST-10).
• Refer members to appropriate behavioral health programs as needed.
• Assist members in closing quality gaps by helping with provider appointments, testing, and referrals to community resources.
• Meet members in community settings (home, shelter, hospital) as needed, acting as an extension of the care team.
