Cityblock Health in Raleigh, NC is hiring a Care Coordinator to support members with complex social, behavioral, and physical health needs. You will collaborate with RN Care Managers, manage a caseload, and develop care plans in partnership with members to address social determinants of health.

The role emphasizes documentation, communication, and timely referrals to community resources, with field visits across home, shelter, hospital, and community settings.

Qualifications

• 2+ years of progressive experience in care coordination, community health, health promotion, or a related field.
• Excellent written and verbal communication; strong documentation skills.
• Must have a valid unrestricted driver’s license.

Responsabilités

• Collaborate with RN Care Managers to support members with complex medical and behavioral health needs.
• Manage a caseload and address social, behavioral, and environmental barriers to care.
• Develop and implement care plans with members to achieve health goals.
• Conduct assessments and refer to behavioral health programs as needed.
• Assist with provider appointments, lab testing, and referrals to community resources.
• Meet members in community settings and document interactions in care platforms.
